With this indemnity form, I exempt Swimsational Swim School, any represented member, any employed member, and the owner of the property or any other responsible person involved with swimming lessons provided by the swim school, from any claims or costs incurred due to injuries, losses, damages, or medical expenses.
I declare that the swimmer is physically fit to attend swimming lessons and does not pose any threat or risk to either themselves or the other swimmers.
I authorise the swimming teacher or any other first aid qualified members or staff to perform first aid medical treatment should it be deemed necessary. Should any medical treatment be required, all costs will be the responsibility of the parent/legal guardian named below.
I acknowledge that in the event of my child ignoring basic safety instructions, they will be removed from the swimming pool area and their lesson forfeited.
I understand that missed lessons will be forfeited with the exception of ONE make-up lesson, which can be booked per month.
I understand that in the event of lightning or unsafe weather conditions, all lessons will be cancelled.
I accept that in the event of a teacher cancelling due to sickness, a substitute teacher will be provided. However, in the event of us not being able to find a suitable substitute, lessons will be rescheduled.
I accept that should I wish to discontinue swimming lessons, one calendar month's written notice must be provided to the accounts section of Swimsational. I further understand that I am responsible for these fees during this notice period regardless of whether or not we attend the lessons during this notice period.
Acknowledgment and Agreement
I acknowledge that I have read, understood, and agree to the terms and conditions outlined in this indemnity form.
Swimsational Swim School Fee Terms and ConditionsUpfront Payment: I understand that all fees are paid upfront for the month ahead. In the event of late payment, I will forfeit my lessons until payment has been made. Should my account fall behind by one month, I will lose my space on the timetable as this will be made available for others to book. Failure to communicate or make a payment plan will be assumed as one month’s calendar notice, which I will be liable for.Monthly Invoices: Monthly invoices are sent out on the 24th of every month, and fees must be paid in full by no later than the 1st of each month.Discontinuation of Lessons: Should I wish to discontinue my swimming lessons, one month's written notice must be provided to the accounts section of Swimsational. I understand that I am responsible for these fees during this notice period regardless of whether or not we attend the lessons during this period.Fee Calculation: The per lesson rate is multiplied by 42 lessons per 12-month cycle and divided by 12 months. Therefore, some months work out cheaper than if I were to swim at the per lesson rate, which justifies why months where the school is closed for breaks still require full fees.Rejoining Fee: In the event of stopping lessons and rejoining at a later stage, a non-refundable R500 rejoining fee will be charged. This fee helps cover losses incurred when a client chooses not to swim over a month when the school is closed for a break.Debt Collection: I understand that failure to settle my account and/or communicate a payment plan may result in being handed over to a debt collector. From that point, Swimsational may no longer intervene.I acknowledge that I have read, understood, and agree to the fee terms and conditions as well as the indemnity clauses outlined in this document.
